When a child falls, gets a bump or a bite, or starts to feel unwell, write it down the same day. The parent sees exactly what happened and what you did, and you have a record in the child's file if anyone asks later.
The form has room for the child's name and age, the date, time and place, the type of incident, what happened, the body part and first aid given, how and when the parent was told, and signatures from you and the parent.
Print at 100% or fit to page. Print as many copies as you need for your own daycare. Give the parent a copy and keep one. Your state's licensing rules may say which incidents you must report to them and how fast, so check them too.
